Complex Cases Referral

This form is to submit a referral for Complex Case Management, located at Denver Rescue Mission’s Lawrence Street Shelter. This referral is open to service providers in the city of Denver working with clients interested in exiting homelessness who are struggling to complete case management tasks and directives independently and through traditional pathways. If approved, this team will partner with your agency to navigate the needs of the client you refer.

To submit a referral, complete the documents and requirements below, and attach them to the form at the bottom of this page.

• Confirm that your client is literally homeless
• Confirm that your client desires assistance with exiting homelessness
• Confirm that your client meets our qualification criteria (listed below)
• Complete and upload Denver Rescue Mission’s Release of Information
• Complete and upload your organization’s Release of Information (ROI) if available
• If the client has been submitted to the single entry point (SEP) for long term care, fill out the RMHS Release of Information (if applicable)
